Abstract
The purpose of this study was to analyze Company XYZ’s Digester Department’s confined space situations, associated procedures, and current equipment to identify areas where potential loss may occur in the event of an entry related emergency. Utilizing the Environmental Protection Agency’s confined space survey form in conjunction with Company XYZ’s confined space inventory list, a site survey was conducted to determine the presence of permit-required confined space situations within the organization’s Digester Department. Through interviews with Company XYZ employees, information regarding the presence of situation specific entry procedures and the equipment needed as well as what was available for confined space entry activities was obtained. The data collected identified that Company XYZ’s confined space inventory was outdated and required revision on the classification of confined spaces within the Digester Department. Additionally, through interviews, it was found that current entry procedures contained inaccurate information and thus required revision to reflect current practices. Study-based recommendations to mitigate risk levels associated with Company XYZ’s confined space entry practices include updating of procedures which were identified to contain inaccurate information, periodic surveying of permit-required confined spaces present at the facility, as well as an increase in the current inventory of required entry equipment.
