Abstract
The objective of this study was to assess various risks that were associated with Company
XYZ’s artificial intelligence-based warehouse facility installation project. This assessment
consisted of an employee survey as well as on-site reviews of confined spaces entry, fall
protection, housekeeping practices as well as the collection of airborne carbon monoxide
monitoring data. The survey identified problems with ventilation and air quality, ergonomic
issues on transfer deck, and an absence of standard training. Carbon-monoxide levels remained
below OSHA and NIOSH exposure limits, although concerning, readings were detected at higher
elevations when fossil fuel-powered equipment were being used, which suggested that limited
facility make-up air was present. The confined space assessment for Company XYZ determined
that the transfer deck was a non-permit required space, although there was an absence of
necessary entry protocols and visible warning signs which indicated administrative deficiencies.
Fall hazard risks such as inadequate guardrails, open platform edges, and missing inspection tags
on employee harness-based devices were identified. Overall, Company XYZ’s housekeeping
situation was satisfactory, although several instances of floor debris and unsecured tools were
noted. Practical recommendations for Company XYZ include the implementation of systematic
training in safety as well as ventilation during equipment-related work and considerations with
respect to ergonomic issues within confined areas. Company XYZ would also benefit from a
confined space entry program and written inspection procedures for all fall-protection systems.
The findings indicated that systematic risk-assessment methods are necessary during complex
warehouse installation projects.
